Please share if you have info: my copy has a slipcase that appears to be identical to the copy shown in this European release from 29 NOV 2004 (specifically, see photo 4):

My question - does anyone else have a copy of this disc and is the slipcase from Europe or is it from Japan (or elsewhere)?
